All Athletes and Parents of Athletes

In order to participate in practice, games, or events, ALL online forms must be complete prior to the first practice. Incomplete forms will result in sitting out of practice until complete.

ALL FORMS:

Skyward: All Athletic forms are located on Skyward Family Access.

1. Sign in to your Family Account

2. On the left tab bar, click on "Online Forms"

3. There are 7 forms to fill out and sign. You will get a green check mark when complete.

4. Physical Forms are due every other year. This is a form that must be signed by your physician. If you are able, please upload or attach your athletes physical form signed by a physician. Paper copies are also accepted. Please drop these off in the District Office or give to the Athletic Director during the summer or bring to your first practice.

5. Alternate Year Cards no longer need to be turned in as it is the #5 form that you will fill out and sign in Skyward.

Physical examinations taken April 1 and thereafter is valid for the following two school years; physical examination taken before April 1 is valid only for the remainder of that school year and the following school year. VMH Forms

Your athlete has been sent an email to their school email from the Athletic Trainer. Follow the instructions in the email from the trainer. Here you will complete 2 forms. "Health History" and "Permission to Treat". The email will come from noreply@athletictrainingsystem.com not the trainer personally.

*If your athlete has not received an email from the trainer, please reach out to the Athletic Director*
